Detailed process for Senior High School

  1. INQUIRY: Inquiry is sent through different channels and parent needs to send the following basic information to [email protected] to start the process: Name, Email, Grade Level Applying To

  2. APPLICATION AND REQUIREMENTS: Parent receives the tuition fee, admissions forms to fill out, and requirements to collect and submit. See below.  

Admissions Requirements for G1-11: 

CCF-LAI Forms

  • Student Application Form

  • Teacher’s Recommendation Form

  • Guidance Recommendation Form

  • Student Developmental Form

  • Pastor’s Recommendation Form

  • Student Medical Record

Non-Filipino Applicants

  • Passport (Student/Parents-photocopy bio-page and latest admissions with valid authorized stay)

  • ACR I-Card (Student/Parents-photocopy of front and back portions)

Other Requirements

  • 2X2 ID Picture of Student

  • 2X2 ID Picture of Parents or Guardian

  • Form 138 (2 Previous Years of  Final Report Card and Current Report Card)

  • Certificate of Good Moral Character

  • Birth Certificate

  • Marriage Certificate

  • Immunization Record

  • Grade 9-11: Medical Certificate with Drug test result and chest X-ray result (taken within six months)

  • Grade 10-11:NCAE Result

  • Grade 11 – Photocopy of Passport bio page

3. ASSESSMENT: Once all forms are filled out and requirements are submitted to our Admissions Officer, the student will be scheduled for an assessment onsite.  

Assessment Fee: PHP2500

4. GUIDANCE INTERVIEW: The student will proceed with the guidance interview after passing the entrance exam. 

5. FINAL INTERVIEW. This will be conducted after passing the guidance interview. This interview will be attended by both student and parent. 

 

6. WELCOME PACKET: A welcome packet will be issued to successful passers containing reservation guidelines, covenant forms, and admissions agreement. The parent will also start receiving updates about enrollment.
